Professional referrals
We welcome referrals from professionals.
- If you would like to refer a young carer (under 18), please use the young carer referral form .
- If you would like to refer an adult carer (18 and over) please use the adult carer referral form .
- Please ensure that you have the carer’s permission to pass their contact details and other information to Carers Link.
Because carers are under stress they don’t always know why they’ve been referred so it’s always useful if you can include what you think the carer is in need of (e.g. Adult Carer Support Plan/ Young Carer Statement, social groups, condition specific support etc) as this provides a good starting point when we contact them.
Once we receive the referral we will contact the carer (normally within 3 working days) to seek their consent to store their details, register them on our database and find out more about how we can best support them. Each carer also receives a welcome pack and a carer ID card.
Once they are registered, the carer will be allocated to a carer support worker who will normally make contact within a week. When the staff member makes contact, they will discuss a bit of a case history with the carer – their current situation and concerns they have. They will then move on to helping the carer to identify what support they need, and discuss what support is available and either directly refer the carer to this or signpost them.
